Eligibility: Brief intake form required for first-time visitors. Must bring photo ID and proof of residency for each member of household.   • Notes: Eligibility: Serves elderly homebound residents of the Town of Washington and the Village of Millbrook, and when able to those in neighboring areas who do not have access to a similar program. May be able to help those with short-term need, i.e. hospice care or those recovering from surgery at home.
